I know one person on this B.B. with AB has had a successful PCA surgery with Dr. Berke. My second Botox shot on Monday doesn't seemed to have helped significantly either. I was wondering if anyone else has considered this surgery and here's a link to the abstract posted by Shaw GY, Sechtem PR, Rideout B. I will give it a few more attempts to increase the dosage... I haven't heard much about this procedure. I've read and been told thyroplasty alone for ABers usually doesn't work but combined with a myoplasty, maybe?

Candace -

Do you know exactly what is done to the PCA with the myoplasty? I may be getting caught up in symantics here, but it was my understanding that the Berke AB surgery involved myoectomy ("plasty" = repair; "ectomy" = removal by cutting.)

One of the benefits of the medialization thyroplasty is the fact that it is reversible (the injected substance can be removed.) I don't know if this is true if the myoplasty is performed (depends on how the "repair" is done, I guess.) A myoectomy is not reversible.

Hi Robin and Candace,

And, to make semantics even more complex, since several of us were asking this question back in 2000-2001 (difference in terminology and procedure for this AB surgery), the person Candace may be referring to actually had a Left Partial PCA "Myotomy" (so Berke terms it, rather than a "myectomy") and was the first one to have that procedure.

A couple years later (by the time we all met in Arizona for the 2002 SD Symposium and Colleen's surgery was highlighted there, by Dr. Berke himself) there had only been one or two more. Possibly because it is not reversible and the thyroplasty procedures (reversible) were gaining more attention by 2001-2002?

Thanks, Lynne. I wonder why this surgery has not been studied more. Now that many doctors believe thyroplasty for AB patients is a waste of time, I would think this "myotomy" or "myectomy" would be back in the forefront again. Of course the irreversible nature of the surgury probably has much to do with this.

I just went back to my 2001 Scottsdale NSDA Symposium notes and checked. The surgery was referred to as "a cut of PCA muscle." So that would indicate "myotomy" rather that "myoectomy". No removal involved I guess. Now I really wonder what they mean by "myoplasty"!
