I had to look up thyroplasty. After doing some reading on thyroplasty it seems to me that it is to treat vocal chord paralysis. Isn't that different from spasmodic dysphonia? Did someone suggest this would solve your voice issues? I'd definitely explore all other options including voice therapy with a good speech pathologist before undergoing this surgery which is permanent. You may find that it creates other issues and may or may not successfully treat your ADSD. Good luck and keep us in then loop. Cynthia |

I had the surgery in May 2014 (Thyroplasty Type 2). You may find my comments if you search back in this forum to April/May 2014.
It's now 2 1/2 years since the surgery. Before I had heavy ADSD for more than 6 years, taking Botox all 3 months with very various results. Rating my voice on a scale fro 0 (situation pre-surgery) to 10 (absolute perfect voice), I would give a 8, which means: no more spasm, no breathing problems, but still a bit raspy voice. I take ARTANE, an old Parkinson med without any serious side effects (and not habit forming like Baclofen or other benzos) and very low cost; without the pills I rate my voice 6-7, which is still much better than before the surgery. The surgery is quite simple, but needs experience. In one sentence: They open your neck (front) and place a very small titan bridge between your two voice chords. Result: the voice chords can not cramp together anymore, you can speak again without the effort you needed before. It does the same like Botox, but without killing nerves or reducing your voice capabilities. There is a broad scientific literature covering this type of surgery, including empirical studies discussing long-term outcome. Empirically evaluated long-term success rate is about 70% for men and 80+% for woman (google for Thyroplasty type 2, not for thyroplasty alone). Unfortunately you will hardly find an experienced surgeon for that in the USA or Europe. But in Asia and especially in Japan this surgery is very common for SD patients (main reason: Botox is very expensive there). Compared to SLAD, this surgery is much less risky, because it is non-invasive, needs only 45 minutes with local anesthesia, only one night at the hospital, can be repeated and reversed. Theoretically minutes after the surgery you could use your voice again, but they ask for a silent period of one week, due to the wound in your neck and the stress for your vocal chords. You can find a video on Youtube, recording and explaining such a surgery. In Japan the are doing it at a fixed all-in price of 5000 US$). My surgeon has done it about 500 times over the past years. For me it was the right decision. I do not need Botox any more. I'm able to speak for hours (I'm a lecturer) an can use my mobile without amplifier or other help. Please feel free to ask if you need some special information. But first check the web - you can find many papers covering this subject. |

Very interesting. I will ask my doctor when I see him again in December for my botox treatment. Trouble with getting an appointment and being rather new to this, is I waited too long..haha. My voice is starting to break again. Ugh, I wished this surgery was something he can do. He did tell me he has performed surgery on a few patients. It sounds hopeful and would love to have a permanent solution work for me. Thanks!! |

I am trying to find information about the Thyroplasty II procedure, and I have viewed UTube videos and read several websites from Japan promoting this procedure. I don't understand why it isn't a more popular choice. Are you still happy with your voice after three years? Have you had any complications? Thank you for your reply. |

The most current research report from the Isshiki Voice Center in Kyoto (published in the NSDA current newsletter, pages 12-13) reports over 500 ADSD patients in the past 5 years. Without indicating the response rate or long term follow up, the author states: "60% of patients were very satisfied, 30% felt reasonably satisfied and 10% were unsatisfied with the results." Also, "this surgery is not appropriate for abductor SD." There is contact information at the end of the article. "The NSDA has been working with Nobel Pharma to bring these bridges to the US for a clinical trial for ADSD". Currently some US doctors indicate they offer "Thyroplasty" - it is not clear to me how this differs from Thyroplasty Type 2. More mystery, unfortunately. Better information would be helpful! |
