I know, I keep putting the "h" in chords, while most people don't. It's a habit (and an acceptable spelling). Well, here's my question: I've noticed posting from many of you in which you mention having injections in both chords at the same time. My doctor won't do that in his office; he fears that my chords would close and I'd get no oxygen. I have had both sides injected while in the hospital and under anesthetic, however. Does anyone else have a doctor who will not inject both chords during the same treatment session? I'm curious. Thanks. Larry |

Hi Larry.. The otolaryngologist I see will inject both chords(cords) but prefers to do just one side. Actually the results are just as good and there's very few of the side effects...choking, Mercedes MacCambridge voice, etc. Gavin Hadley |

When I was getting botox, my doctor injected both cord routinely. Never had any breathing problems afterwards. I was AD/SD. John |

Larry, Shortness of breath is a common side effect when injecting the abductor muscles (the PCA's) because those are the muscles we use to open the airway to breath, so weakening them with BOTOX restricts the airway. Most doctors will not inject both sides at once in AB/SD patients, or at least not until the patient's response to BOTOX is well characterized. I have never heard of that being a concern with injections for AD/SD. Based on your previous posts I thought you had AD/SD, it that correct? Bill Vanderlinde

I believe that I have AB. You'll have to excuse me for not knowing all the terminology--I'm still new to this strange phenomenon. What are PCA's? |

Larry, PCA stands for posterior crico aritenoid. There are two PCAs, one on the left and one on the right. They pull the vocal cords apart when you breath in, this action is called "abduction" by the medical professionals, so the PCA's are the "abductor muscles". If you have "abductor" SD then these are the muscles that are over-active and you can't properly close the vocal cords to produce certain sounds such as h,s,p (the unvoiced consonants). So the typical treatment for AB/SD is to inject the PCAs so you can close your vocal cords more easily. However, you need to have some activity in at least one of the PCAs so you can open the airway for breathing. So doctors tend to be very careful about injecting both sides at once. The first two years I was injected I received injections in alternate sides every three months, and the doctor would check with a laryngescope to make sure I still had some motion in one side before he injected the other side. Once he had a real good baseline on my response to BOTOX, he started injecting both sides in the same visit. Best,

Bill, Met with my doctor last week and he clarified for me that I have AD/SD. He cautioned, however, that "labeling" is not an easy thing, and mentioned that many SD'ers have some elements of both AB and AD. Well, I get another Botox injection on Wednesday. I think he's planning 15 to 20 units (though I'm not sure what a unit is).
